Help at Home Care: Commonwealth Home Support Programme

The Commonwealth Home Support Programme provides a variety of entry-level, help at home, support services to benefit older people and their carers to stay independent and in their homes and communities for longer.  The programme is focussed on a restorative approach incorporating wellness and reablement to help at home services.

As part of the Home Support Programme, Anglican Care provides the following services:

Alkira Respite & Day Therapy Centre, Toronto

Anglican Care’s Alkira Respite & Day Therapy Centre is specifically designed for seniors to provide an interactive and social environment.  People can meet new friends and enjoy a program that promotes and maintains health and wellbeing whilst having fun. The Centre provides various types of therapies to help seniors to maintain independence and mobility for as long as possible.

The main services provided by Alkira Respite & Day Therapy Centre include:

  • Social support – both individual and in a group environment. This can include: community outings, games and concerts
  • Allied health and therapy services – such as podiatry (foot care), physiotherapy (exercise, mobility, strength and balance assessments and tailored programs to suit the individual) and services of a dietician (nutrition assessment, food and nutrition advice and dietary changes)
  • Exercise program – group exercises to keep you physically well and active
  • Meals – morning tea and a two course hot lunch are provided
  • Personal care
  • Carer support
  • Networking and liaison with Health Care Services
  • Transport – Anglican Care can provide transportation to and from the Centre on a daily basis.

If you have a loved one who is interested in attending the Alkira Respite & Day Therapy Centre, you or your family/friends can contact us directly to discuss attending the Centre. You will then require a home support assessment (conducted by the Regional Assessment Services (RAS)).  To find out more information about the home support assessment you will need to contact myagedcare on 1800 200 422.

One of Anglican Care’s staff members will then conduct an initial assessment to identify the types of therapy needed and to work out what you want out of the therapy (your goals). They will also work out an individual action plan for you based on these goals.

The Australian Government funds the Commonwealth Home Support Programme service providers so they can deliver affordable services to people. There is a fee for each service and the amount people contribute depends on their income and the type and number of services required.

What you will pay to attend the Alkira Respite & Day Therapy Centre will be discussed and agreed upon with you prior to commencement at the Centre.

Allowances and subsidies are available for those families who are financially or socially disadvantaged and these can be discussed with the Day Therapy Centre staff.

Help at home

East Lake Macquarie Dementia Service – Belmont

ELMDS offers centre-based care in a home like and secure environment for people living with dementia.

The aim of the Centre is to maintain people’s social and self care skills while promoting an individual sense of wellbeing.

Programs and activities are designed to suit the differing stages of dementia and include:

  • Craft
  • Pool/snooker
  • Barbeques
  • Concerts
  • Card games
  • Fishing group
  • Mixed Days
  • Pamper sessions
  • Cooking
  • Painting
  • Gardening
  • Regular Carer Social Outings

For carers we provide respite care to enable you to take a break and complete all those tasks it’s hard to find time to do.  We also provide the opportunity for carers to mix with other carers on our very popular social outings.

Emergency respite care is also for those unexpected times when you may need assistance.  This may be for a day or for a short period so you can attend appointments.

Our Younger Onset Dementia Client Program is for people aged up to 66.  This program allows us to encourage socialisation and independence with a group of like ages and interests.  We understand that younger dementia clients often have differing interests and this program enables us to offer smaller group outings that suit younger clients living with dementia.
